There are two main "routes" through the stepped program and information about the two routes is included.

• ALL patients of any age or any origin who have type 2 diabetes
• Patients under 55, who are not of Black African or African-Caribbean family origin

• Patients aged 55 and over, who do NOT have type 2 diabetes
• Patients of any age who are of Black African or African-Caribbean family origin, and who do NOT have type 2 diabetes
